Cause

Approval flows auto-provision a Dataverse database on first run; that provisioning is still in progress or has failed.

Fix

In Power Platform admin center, confirm the environment's Dataverse database shows Ready. If provisioning failed, recreate the database or open a support ticket. Once Ready, rerun the flow. Ensure the running user has a Power Automate per-user/per-flow license to satisfy provisioning prerequisites.
